Company Name: Enter the name of your practice.
Client Name & Email: Enter the information for the client receiving access.
Partner Code: Enter your practice’s unique code in the coupon code box below. This will reduce today’s balance to $0. Your practice will be billed separately according to your partnership terms.
Your client will automatically receive their Built To Birth login credentials by email.
Client Didn't Receive Their Login Email?
Try This Quick Fix
Click the Reset button below and enter the same email address you used to enroll the client. This will send a password-reset email directly to the client so they can create a password and access their account.
Still Didn't Receive An Email?
There may have been a typo in the email address used during enrollment. Email [email protected] with your company name, client's name, and client's correct email address, and we'll take care of it.
Please Don't Enroll Your Client Again
We'll correct their existing account so you don't end up with a duplicate enrollment.
