Company Name: Enter the name of your practice.
Client Name & Email: Enter the information for the client receiving access.
Partner Code: Enter your practice’s unique code in the coupon code box below. This will reduce today’s balance to $0. Your practice will be billed separately according to your partnership terms.
Your client will automatically receive their Built To Birth login credentials by email.
Need help with an enrollment?
If a client hasn't received their login email, have them check spam, junk, and promotions first. If it's still missing, email [email protected] with your company name, client's name, and client's email address. Please don't enroll the client again.
